Instructions for filling out an application for a refund of freight charges in conditions of COVID-19
The application for a refund must be filled out by the passenger or the parent/legal representative of the minor.
To receive a refund for an air ticket/certificate during the COVID period, the passenger must fill out and submit an application form, namely:
- 1. Last name First name Patronymic (in full).
- 2. Date and place of birth of the passenger.
- 3. Citizenship of the passenger.
- 4. Series and number of the passenger's passport.
- 5. Place of registration of the passenger (as in the passport).
- 6. To contact the passenger, be sure to provide a contact phone number and email.
- 7. Number of the air ticket or certificate issued against the air ticket.
The airline ticket or certificate number starts with 262 and then contains 10 digits (for example, 262-1234567890).
If the air ticket number field is filled in, then you must fill in the “flight date” and “passenger’s full name” fields. - 8. Next, you need to fill out the recipient’s bank details for a refund according to the application form. If the passenger wishes, it is additionally possible to attach bank details from the bank via the “Add file” button.
- 9. If the passenger is not the recipient of the payment, you must fill out a consent form to transfer funds to another person and attach the document using the “Add file” button. If the passenger is under 18 years of age, consent from a parent/legal guardian is required. Each passenger fills out the consent form!
- 10. If the certificate is issued on the basis of several air tickets, then each passenger indicated in the air ticket must fill out a consent form for transferring funds to another person and attach the document using the “Add file” button.
- 11. Attach a copy of the document (passport/birth certificate) for each passenger to the application
